Medical Management Team (Case Management)

The Medical Management Team includes case managers, wounded warrior case manager, and disease manager located in Building 1465A. A utilization manager is located in building 1465 in the TOPA area. This service is active duty only and the goal is to help manage/coordinate care for complex patients and ensure appropriate resource utilization.
